The five-minute caregiver handoff

Put the important details in one place before care changes hands.

Part 1

Share the essentials

Cover today’s routine, the last feed or meal, sleep setup, comfort cues and how to reach the parent. Include allergies and clinician-provided instructions when relevant. Never rely on memory for a medication plan.
Part 2

Walk through the space

Show where supplies, spare clothes and emergency information are kept. Demonstrate any equipment the caregiver has not used. Ask what still feels unclear.
Part 3

Agree on the return update

Choose a short update format: what happened, anything concerning, and what needs restocking. Avoid collecting more private information than the caregiver needs.
Use our printable handoff sheet for the conversation. Keep completed sheets private and dispose of outdated copies. The online worksheet does not save what you type.

One thing to try today

Print a blank handoff sheet and fill it out with the next caregiver.

For a partner, grandparent or helper

Repeat back the plan and ask questions before the parent leaves.
